Quality Improvement Program

Coordinated Care’s Quality Improvement (QI) Program focuses on the health of our members. The QI program looks for ways that we can help improve your health and safety. Our goal is to make a difference in your health. We also work to make sure you and your doctors are happy with us. The QI program includes providers from our community to help us do this.

The Quality Improvement Program also reviews services provided to our members, as well as suggestions from members like you!  We want our members to tell us how we are doing with our services and how we can better assist you. We follow national guidelines for treating some conditions to better help you live well with your conditions.

Our work is aimed at helping you. We can help people with diabetes get their yearly eye examinations. We can help families with children get their well visits and vaccinations.  We have tools to help you work through hard times and build brighter futures.  And we host community health education events. The QI Program is involved in making sure all our materials and interactions with you are in forms that are understandable and in the language you need.
