We appreciate your collaboration as a partner with the Coordinated Care provider network. The tools in this section allow you to navigate the following:

Add Provider to Group

• Use this to add a provider (practitioner) to an already contracted group/location.
  

Add Behavioral Health Provider to Group

• Use this to add a behavioral Health provider (practitioner) to an already contracted group/location.
  

Demographic Update Tool

• Use this to update location(s) and provider demographics such as location address, phone numbers, office hours, term a location, term a provider from a group, panel status and more.

Term an Existing Provider

• Use this to term a location or term a provider from a group.
