Gender-Affirming Care Management

As a Coordinated Care member, you can choose to enroll in care management to get support exploring or pursuing gender-affirming services. Care management is confidential and is one of the no-cost benefits of being a Coordinated Care member.

Our care managers can help you navigate your Apple Health benefits coverage. They are here for you during your health care journey. You're covered for:

• Puberty blockers
• Hormone therapy
• Surgical services
• Mental health services
• And more

Gender-affirming surgeries require paperwork and approval from a variety of healthcare providers. A care manager can help you collect and prepare any paperwork and provider reports, so each part of the process is a step in your journey and not a barrier to care.
