Coordinated Care Mobile App: View Health Information on the Go

Our members often have trouble accessing simple pieces of information about their health plan when they don’t have access to a computer. Whether it is viewing their Member ID or finding their provider contact information, many barriers can make access to care more difficult than necessary.
These issues can also make it harder for you to care for your patients. But you can help empower our members to access their health information by downloading the Coordinated Care Mobile App.
Just have them search for “Coordinated Care” on their Apple or Android phone or tablet through the App store. The members will need their online member login information to access their account. In the App they can access features like their electronic Member ID, PCP Contact Information, and their care gap Health Alerts.
If members have forgotten their password or do not yet have an account, they can go to our website and click, “Login,” and follow the instructions to set this up. If they need help with an account setup, they can call Member Services at 1-877-644-4613.
