Addressing healthcare inequity from every angle, we improve individual quality of life and bolster the state’s crucial provider network. We’ve designed innovative programs, solutions and support systems that break down barriers to care, improve access and reduce health disparities.
Healthcare Equity
Social Drivers of Health (SDOH)
The environments where people live, learn, work and play are critical factors in their overall well-being. A whole person care mindset acknowledges the importance of these social drivers of health (SDOH).Since 2020, we invested over $4 million in programs focused on improving SDOH for our members and communities across the State.
Food Insecurity
Consistent access to healthy food is one such necessity, which is why we've pioneered multiple pilot programs to address food insecurity in Washington. Our fruit and vegetable prescription program, Coordinated Care Harvest Bucks, was the first of its kind in the state. Our One Million Meals program has provided 9+ million fresh, nutritious meals to Washingtonians since 2020.

Housing
Safe, affordable housing is another critical SDOH. Together with our community partners, we’ve created emergency assistance programs to support housing needs for those with low-income. Over the last few years, we worked directly with Mercy Housing to develop six affordable housing properties in Tacoma and Olympia.
Foster Care
Our goal is to break down barriers to whole person health and expand access to care for the state’s most vulnerable people. As the only health plan serving the child welfare population in Washington, our expertise is unmatched. Through our Apple Health Core Connections (AHCC) program, we care for 21,000+ children and young adults in various stages of the foster care system. We provide reliable, consistent healthcare services for these children, families, and caregivers during a volatile time in their lives.
Coordinated Care donated over $380,000 to organizations such as Treehouse, Sibling Strong, The Mockingbird Society and others that support youth in foster care. Funding from Coordinated Care enabled Sibling Strong to expand its programs for siblings separated by foster care in King and Pierce Counties. These programs help to eliminate barriers to participation like transportation, and bring siblings together in a fun, safe environment.
Maternal Healthcare
No two pregnancy journeys are the same. Coordinated Care offers customized care and specialized resources to support members and their babies every step of the way. We partner with First 5 Fundamentals’ Family Connect program to expand postpartum nurse support, and we were the first plan to offer 24/7 access to virtual doula services to our members.
Our maternal health programs are designed to address SDOH during pregnancy and childbirth when it’s critically important to ensure a healthy start. For members who struggle with addiction, our Healthy Beginnings pilot program offers immediate access to substance use disorder treatment to help improve health outcomes. We also helped fund Maddie’s Place, a recovery nursery designed especially for babies experiencing withdrawal due to prenatal substance exposure.
Our Start Smart for Your Baby program provides members with necessities like a no-cost car seat and breast pump, as well as access to a case manager for extra support.
Behavioral Health
Whole Person Healthcare
We know the best way to care for our members is to address their needs holistically. When members seek care, they bring more than just their physical symptoms. They bring behavioral, economic, and social conditions as well. A complete picture of health includes all of the interconnected factors that contribute to wellbeing in the long and short term.
Our whole person care approach bridges the gap between physical and behavioral healthcare and it’s the foundation of each of our local programs, including Choose Tomorrow.
Choose Tomorrow is an innovative suicide prevention protocol designed to quickly identify and assess members at risk of self-harm. We provide both immediate and ongoing services for members, including outreach, screening and safety plans.
Our value extends well beyond our direct services; we put our expertise to work for local partner organizations. We share with them our evidence-based approach to address complex behavioral healthcare needs. Coordinated Care is the only plan in the state to offer free training to providers, partners, and caregivers on topics like mental health first aid and trauma-informed care.
Investing in Our Washington Partners
Through our ever-growing network of providers, Coordinated Care is expanding access to critical behavioral healthcare services across the state.
In 2023 we partnered with 80+ community organizations and contributed 600 employee volunteer hours in the state. This adds up to an impact of $1.8 million of community reinvestment.
Additionally, we support the recruitment, training, and retention of behavioral health workers across the state through our partnerships with over a dozen community and provider organizations. These partnerships help direct dollars where they are needed most in neighborhoods across the state – targeting the priorities of those who know their communities best.
Capacity Building
Workforce Development
Our goal is to break down barriers to whole person healthcare and expand access statewide. An agile, future-ready healthcare workforce is essential. Over the last three years, we’ve worked with 23 organizations to support nearly 500 students pursuing healthcare careers across the state.
Community Scholarships
Since the program launched in 2021, we expanded our Coordinated Care Community Scholarship program by 400%. In partnership with community and technical colleges and other organizations across the state such as Clark College, Tacoma Community College, Peninsula College and the Therapy Fund Foundation, we provided financial support for first-generation and BIPOC individuals pursuing behavioral healthcare professions.
Rural Healthcare Initiatives
With the greatest geographic breadth of any managed care partner in the state, we understand the unique healthcare challenges of rural communities. Coordinated Care leverages our community partnerships to meet our members where they are. We offer both telehealth services and person-to-person outreach to deliver high-quality healthcare services to our rural patients.
