Foster Care Center of Excellence designated by Coordinated Care

“Foster Care Center of Excellence” (FCCOE) is a designation Coordinated Care gives to medical centers that provide expert, trauma-informed care for child welfare-involved families. They provide a high standard of care and access to resources. To receive this designation, medical centers must meet more than 50 criteria.

At a Foster Care Center of Excellence, children and youth in foster care can see high quality primary care providers. These providers understand the unique needs of families and members of Coordinated Care's Apple Health Core Connection program. The doctors and staff partner with parents and caregivers to ensure the wellbeing of the kids in their care.

An initial 5-day health risk exam and an EPSDT within 30 days of entering foster care.
 

Behavioral health services like Individual Therapy, Group Therapy, and Cognitive Behavioral Therapy.


Lab tests and x-rays.


Prescriptions.
 

Referrals to specialists like Physical Therapy, Speech Therapy and Occupational Therapy.
 

Referrals to vision and dental services.
