Dental, Hearing & Vision Benefits

Coordinated Care members have coverage for dental, hearing and vision services with their ProviderOne Card.*

Getting Dental Care

Apple Health covers dental services for eligible children (age 20 and younger) and adults (age 21 and older).

Covered dental services include:
  • Routine exams
  • Cleaning
  • X-rays
  • Fillings
  • Fluoride application
  • Extractions (pulling teeth)
In addition, Coordinated Care covers:
  • Prescriptions written by a dentist
  • Access to Baby and Child Dentistry (ABCD) services from an ABCD certified provider
  • Medical/surgical services provided by a dentist
  • Hospital/Ambulatory surgery center facility charges

 

You must use a provider that accepts Apple Health. To find a dental provider:

The Academy of Pediatric Dentistry recommends all children see a dentist by 12 months of age.

Pregnancy IconDental care and treatment during pregnancy is safe and important. The American College of Obstetricians and Gynecologists and the American College of Nurse-Midwives recommend that pregnant people get regular dental check-ups and have cavities and other dental problems treated right away.

Hearing Icon 

Hearing Care

Hearing exams and hearing aids are covered for Coordinated Care members.

  • Routine hearing exams and screenings are covered for all Coordinated Care members.
  • Hearing aids are covered for children 20 and younger and adults who meet program criteria.
  • Cochlear implant devices and Bone Anchored Hearing Aid (BAHA) devices are available for children 20 and younger. Replacement parts are covered for anyone who already has an implant.

Vision Care and Eyeglasses

Eye exams, glasses, and contacts are covered for Coordinated Care members.

Members Under 21

  • No-cost eye exam once every 12 months.
  • Eyeglass frames, lenses, and contact lenses are covered by Apple Health fee-for-service, using your ProviderOne card.

Members Age 21+

  • No-cost eye exam once every 24 months.
  • $100 discount on eyeglasses or contact lenses every two calendar years with a participating in-network Envolve Vision provider. In addition, standard fitting fees are covered in full.

Adults 21 and over can also get discounted eyeglasses through certain providers. For a list of providers that offer discounted eye hardware, visit HCA’s website.

Coordinated Care also offers the New Eyes Program, which provides access to free lenses and frames. Members with care management can talk to their care coordinator about the program.
