Coordinated Care offers many convenient and secure tools to assist you. You also have access to your healthcare information. To enter our secure portal, click on the login button. A new window will open. You can login or register. Creating an account is free and easy.
By creating a Coordinated Care account, you can:
- Change your Primary Care Doctor
- Request a new Member ID Card
- Update your personal information
- Send us a message

Provider Portal
If you are a contracted provider, you can register now. If you are a non-contracted provider, you will be able to register after you submit your first claim. After creating an account within the Coordinated Care provider portal you can:
- Verify member eligibility
- Check & submit claims
- Submit & confirm authorizations
- View detailed patient list
